Job Description

The LTSS Care Navigator (LVN) will provide support to members and healthcare partners by conducting follow-up activities, completing required documentation, and ensuring tasks are completed within established turnaround times. This role requires strong communication skills, attention to detail, and the ability to work independently in a home-based environment while maintaining HIPAA compliance.

Key Responsibilities

• Conduct follow-up calls to members, physician offices, DME providers, and other agencies as needed.
• Complete all assigned tasks and meet required turnaround times consistently.
• Track weekly productivity and submit reports in spreadsheet format.
• Perform field visits within Corpus Christi and surrounding areas, if required.
• Complete necessary forms and accurately document assessments within expected timelines.
• Work independently from a home office while ensuring patient privacy and preventing HIPAA breaches.
• Maintain reliable and stable internet connectivity.
• Communicate effectively with members, providers, and internal teams.
• Complete formal training and adhere to departmental productivity and workflow expectations.

Education Requirements

• Active LVN license in good standing with the Board of Nursing (BON).

Required Skills & Experience

• Licensed Vocational Nurse (LVN).
• Home health experience.
• Experience working in a physician/medical office setting.

Preferred Skills & Experience

• Strong verbal and written communication skills.
• Proficiency with Microsoft Office tools (Excel, Word, etc.).
• Ability to learn new systems, processes, and workflows quickly.
